You or your child’s doctor can send an e-fax directly to our email inbox at 608-509-9255. You can also scan and email it to us at lclareservation@gmail.com. We need current, hard copies, of your child’s paperwork in order to be able to care for them in the centers. This is a State of Wisconsin Child Care Licensing Requirement. The required paperwork can be found HERE, or let us know if you need a copy of it, and we can provide it to you in the centers!
