Two weeks ago I sent you your new classroom welcome letters and paperwork. In there were forms for the ACH, Door Codes and UW Affiliation. Every year we have you re-fill these out as we start new in some of our programs that we use. If you could fill these out and send back to me by Thursday that would be great.
